Gift Certificates
Once you have made your online purchase you will receive your Gift Certificate(s)
in a PDF file email attachment within 24 hours. You will be able to download, print, and give!

You can also purchase Gift Certificates in person Monday thru Thursday from 9-12 and 2-5. To purchase gift certificates outside of these hours please call to make arrangements.
Location: 10232 San Jose Blvd. Jacksonville, FL 32257 (one mile north of I-295 across from Bowl America in the Baptist Primary Care Office Park)
Gift Certificates are good for one year from the date of purchase and are non-refunable but can be transferred to another person.
Cancellation Policy: A 24 hour notice is required to change or cancel an appointment. Without proper notice the Gift Certificate will be void.
Cash and Checks are accepted (credit and debit cards accepted online only).
